LPHI staff may work to assess opportunities for care coordination within each state. This will include assessment of current relationships among clinics and hospitals as well as plans for clinics and hospitals to connect to health information exchanges. These assessments will drive the type of Care Coordination technical assistance that is provided. For clinics that do not have immediate plans to connect to a health information exchange, the technical assistance will center around optimizing current processes to receive information from/transmit information to hospital ED and Inpatient settings and to receive information from/submit information to specialty care providers. For clinics that have an immediate plan to connect to a health information exchange, technical assistance will be focused on connectivity to the HIE and EMR user interface development associated with the receipt and transmission of ED/Inpatient and specialty care information.
