The team will provide technical assistance services to support health centers with improving chronic care management and practices. The team will provide the following TA services:

• Assist with identifying evidence based guidelines and best practices related to chronic care management
• Develop customized practice technical assistance work plans to guide progress
• Review and provide feedback on documentation such as, policies, procedures, protocols, workflows, quality improvement plans, etc.
• Share available sample documentation and tools, as needed
  • Provide available information on chronic care management resources such as, medication assistance programs, patient education tools, patient self-management support programs, etc.
