LPHI has contributed to significant achievements in the transformation of the New Orleans health care system. After Hurricane Katrina decimated the city’s infrastructure, including crucial primary and emergency care access point, LPHI helped rebuild neighborhood-based community health centers through the federally funded Primary Care Access and Stabilization Grant (PCASG). All health centers participating in PCASG achieved NCQA recognition as patient-centered medical homes. Beginning in 2013, in partnership with local health care leadership, LPHI established the Greater New Orleans Health Information Exchange (GNOHIE). With collaborative, member-driven governance, the GNOHIE supports local hospitals and primary care clinics to share vital patient information and sends alerts when a patient is admitted to the emergency room or inpatient care.
